Contact form terms

These terms cover the free contact form service at nataliapacierpnik.com. You may read, save or print them before submitting a message.
1. Service provider and contact details
The provider is Natalia Pacierpnik, a sole trader operating as NATALIA PACIERPNIK SPORT & MIND, Polish tax identification number (NIP) 7681770322, REGON 545452266, ul. Czerwone Maki 59/32, 30-392 Kraków, Poland.

2. What the service does
The form lets you send an enquiry about working together or the activities of Sport & Mind. It is free to use and does not require an account. You are only responsible for any ordinary internet connection charges under your agreement with your provider.
Submitting the form does not itself reserve an appointment, order paid training or enter into a contract for such cooperation. Its scope, timing, price and other terms are agreed separately before a commitment is made.
3. Requirements and use
You need an internet-connected device, a current browser supporting standard forms and secure HTTPS connections, and a working email address where a reply can be sent.
Choose a topic, enter your first name and a valid email address, and write a message of between 20 and 5,000 characters. You can correct the details before submitting. If the form reports an expiry or error, refresh the page and check the indicated fields. You can also email directly.
You must not submit unlawful content, infringe other people's rights, send spam or interfere with the website. Provide information relevant to your enquiry. Do not send passwords, medical records or other sensitive information unnecessary for initial contact.
4. Starting and ending the service
The agreement for the one-off, free form service is made when you press the submission button on a correctly completed form and lasts until the system finishes processing that attempt. You can stop at any time before submission by closing the page, without any charge.
A message-accepted notification means the message has been passed to the email system. It does not confirm that it has been read or that an appointment has been agreed. If you experience difficulty making contact, you can use email or telephone.
Spam safeguards may temporarily limit further attempts. Any subsequent response to your enquiry is handled through correspondence. Ending the form service does not automatically end that correspondence or delete the data, whose retention is explained in the privacy policy.
5. Complaints
You may complain about the form service by emailing nataliapacierpnik@gmail.com or writing to the provider's postal address. Describe the problem and provide a way to reply. The incident date is helpful; you do not need to use a special template or supply information unrelated to the complaint.
I will reply within 14 days of receiving your complaint, by email or post according to the contact method provided. This period does not limit any more favourable rights under mandatory law.
6. Privacy and external services
The Privacy and cookies policy explains data processing, retention and your rights. Submitting an enquiry does not require marketing consent.
WhatsApp and social media links lead to services of other operators, governed by their own terms. These terms cover the form on nataliapacierpnik.com.
7. Applicable version and user rights
Each submission attempt is governed by the terms made available at the time you use the form. Later changes apply to future services and do not retroactively alter the conditions of a completed service.
These terms do not exclude or restrict users' rights under applicable law, including consumer rights. Matters not covered are governed by the applicable law, particularly the Polish Act on the Provision of Services by Electronic Means.
